Take better notes from a lesson or chapter

Help me take better notes.

Messy notes or topic: [paste]
Source: [lesson / chapter / video]
What I need notes for: [exam / project]

Ask me up to 3 short questions first if anything important is missing. Wait for my answers.

Then:
1. Cleaned notes in a clear structure from my material.
2. Gaps to fill from the source.
3. A note method for next time (short).
4. Three recall questions from the notes.

Only use what I've told you, leave blanks for anything I haven't given, and don't invent facts, steps or official URLs you're not sure of. UK English.
