Write your self-assessment for an appraisal

Help me write my appraisal self-assessment.

The form's questions: [paste them, with word limits]
My notes on the year: [projects, results, feedback, learning]
Ratings, if I give myself one: [scale]

Ask me up to 3 short questions first if anything important is missing. Wait for my answers.

Then:
1. An answer for each question, in the first person, with specific examples and results.
2. A balanced section on what I'd do differently, if the form asks.
3. Suggested ratings with one line of evidence each, if the form asks.
4. A word count for each answer.

Only use what I've told you, leave blanks for anything I haven't given, and don't invent facts, dates or results. UK English.
